H&S Peer Review Inspection - Introduction
1. The SMD ‘higher risk’ areas (laboratories, workshops, ‘communal’ areas where hazardous substances are handled – e.g.
hazardous waste collection areas) are covered by H&S Peer Review inspections (Note - Lower risk areas (e.g. office rooms)
are covered by each Institute’s H&S self-inspection schedule. Institute H&S self-inspection reports may be requested during
peer review inspections).
2. These H&S Peer Review Inspections fit into the QMUL’s overall H&S Audit & Inspection programme (see pp 9 & 10 of the
QMUL H&S Audit and Inspection Standard at http://www.ohsd.qmul.ac.uk/documents/standard/64341.pdf )
3. To keep to a consistent timetable, SMD H&S Peer Review Inspections are scheduled for the second Tuesday of each
month, for an am (10 am - 12:30 pm) slot. If the date / time are not suitable, then every effort will be made to find a suitable
slot in that week close to the second Tuesday of the month. Hosts - please contact Mark Ariyanayagam
m.r.ariyanayagam@qmul.ac.uk if a change is needed.
4. All inspections are to a maximum of 2.5 hours, inspections to be approximately of 2 hours duration.
5. A Minimum of 3 persons in the inspection team, maximum of 5 persons.
6. At least one member of the inspection team to be a (Lead) Safety Adviser, knowledgeable on the safety requirements of the
laboratory / area.

7. Volunteers for the Peer Review Inspection are sought from Safety Coordinators, Lab / Technical Managers / staff,
Academic and Research staff with an interest in Laboratory Health and Safety (e.g. members of an Institute H&S Committee,
laboratory safety grouping). A volunteer can join the inspection team for a laboratory / area that are not under their Institute’s
control (‘exemptions’ will be made for ‘shared / communal’ (e.g. hazardous waste areas) areas). If you can volunteer,
please email Mark Ariyanayagam m.r.ariyanayagam@qmul.ac.uk, noting which inspection/s you can join.
Preparation for the H&S Peer Review Inspection
8. The local safety rules document as an electronic document (or web link to the document) and 2 sample risk
assessments as electronic documents (or web link / database reference ID if available online) from the lab / area to be
inspected must be sent to Mark Ariyanayagam m.r.ariyanayagam@qmul.ac.uk at least a week in advance by the host
Safety Coordinator (please note normally there won’t be a reminder for this action, other than this document).
9. Required information on entry restrictions, health surveillance / monitoring required for the area, PPE that cannot be
provided by the host or other significant safety measures required for the area should be noted to Mark Ariyanayagam
m.r.ariyanayagam@qmul.ac.uk at least a week in advance by the host Safety Coordinator.
10. Mark Ariyanayagam will distribute the above documents (or weblinks) and any other required information to the inspection
team as preparatory information.
11. The Inspection team should also print out the H&S Peer Review Inspection checklist/s at

12. The Inspection Team to meet at the Nucleus Cafe (for Whitechapel) or The Shield (for Charterhouse Sq) or The Ground
Café (for Mile End) at 9.30 am on the inspection day (or 30 mins before the inspection if the start time is not 10 am) for a
coffee and briefing by the Lead Safety Adviser.

15. The Inspection team will follow instructions from the host on safety (incl PPE) / entry requirements for the inspection.
Standard PPE (e.g. lab coat, eye protection) will be provided by the host.
16. In addition to the above requested H&S documents, it would be expected that H&S documents for the lab / area (e.g. risk
assessments, safety rules & arrangements, safety data sheets, inventories of hazardous substances, local exhaust
ventilation (e.g. fume cupboards, microbiological safety cabinets) test certificates) are easily accessible / available in the
lab for viewing by the Inspection Team.
17. At the end of the inspection time slot, ‘key points’ will be noted verbally to the host/s by the Inspection Team.

18. The Inspection Report is to be drafted by a volunteer from the inspection team using the template at the above webpage,
with input from the rest of the inspection team. Guidance to draft the Inspection Report is also found on the same web page.
19. The Inspection report will normally be submitted to the relevant Institute Director and copied to the Faculty Vice Principal,
within 20 working days from the Inspection date. If there are delays to this schedule, the Institute Director will be informed.
20. Where recommendations to improve safety are identified in the report, an action plan will be requested from the Institute
Director to ensure remedial actions are implemented. The Inspection Report, Action Plan and subsequent Completion
of Actions should be included in the Institute’s reports to the Faculty Safety Group.

Containment Level 3 (CL3) / Genetic Modification (GM) work
21. The Containment Level 3 Peer Review inspections will be primarily based on the laboratory infrastructure requirements. The
Biological Safety Adviser (Mark Ariyanayagam) will also conduct a separate document / safety management audit of the CL3
facility / work (to be arranged separately with the relevant Principal Investigator/s and Institute Lab Management) and will
write the final report/s for this inspection. The specific CL3 inspection checklist is at the web page noted above.
22. For the Blizard Containment Level 3 suite (LG97, LG98, LG101 and LG102), individuals may need health clearance from QMUL
Occupational Health (contact Mark Ariyanayagam to ascertain requirements).
23. Where there is a GM element to any lab’s work, at least one member of the QMUL GMSC will be in the inspection team. The
GM Inspection checklists are at the web page noted above.
24. The GM part of the inspection report will be submitted to QMUL GM Safety Committee for review.

Ionising Radiation
25. Designated Ionising Radiation laboratories (Supervised / Controlled Areas) will NOT be inspected under this Inspection
Schedule (these will be inspected separately under specialist Ionising Radiation inspections arranged by the QMUL
Radiation Protection Manager and Radiation Protection Adviser). It is noted that there may be Ionising Radiation sources
present (e.g. within equipment, as teaching aids) in some non-designated laboratories.
