I hereby authorize Lafayette College to begin, change, or cancel (as indicated) the direct deposit of my pay to
savings or checking account and to the bank as indicated by the bank transit-routing number.

I understand that the entire net amount of my bi-weekly or monthly earnings will be deposited through
Lafayette Ambassador Bank to my bank and the account which I have indicated above. Any further
transaction concerning the above account must be negotiated directly between my bank and myself.
This agreement is to remain in effect until another Direct Deposit Form has been submitted to the
Controller’s Office. All notification for begin, change, or termination must be received by the Controller’s
Office by a deadline date for the month’s payroll. Only one Direct Deposit Form to a bank will be in effect at
any given time. Please notify the Controller’s Office before you close your account out.

INSTRUCTIONS FOR COMPLETION OF
DIRECT DEPOSIT AUTHORIZATION FORM
SECTION I
CHECK APPROPRIATE BOX
 If you want to start direct deposit of your pay, check “BEGIN” and complete Sections I and II of
form.

If you are changing banks, accounts, type of account, check “CHANGE” and complete Sections I and
II of form.

If you wish direct deposit of your pay to STOP, check “CANCEL” then sign the form and forward to
the Controller’s Office.
YOUR L NUMBER:
 Please do not use your Social Security Number for your L Number. Use the number that is at the
bottom of your Lafayette ID. This number is at the bottom of your ID and begins with a “9”. The
“9” should be replaced by an “L” and then the other eight digits. This ID should also be used on your
timesheets.
STUDENT – GRADUATION YEAR:
 Please include the year you will be graduating.
BANK TRANSIT/ROUTING NO.:
 A nine digit number that represents the bank to which your pay is to be deposited. This
number must be verified by attaching a copy of a check marked “VOID” or a photocopy
of a check marked “VOID” or a form from your bank which has all bank information.
BANK ACCOUNT:
 Indicate the account to which the net pay is to be deposited. Please write clearly so that the numbers
are entered correctly. Do not include hyphens.
TYPE ACCOUNT:
 Indicate C – for checking
 Indicate S – for savings
YOUR BANK’S NAME:
 Indicate the name of your bank – the bank in which the funds are to be deposited to.
SECTION II Self explanatory – complete all information.
PLEASE NOTE: Automated Clearing House rules require that all requests to begin or change direct
deposit input must go through a verification cycle to confirm the information completed on this form. No
direct deposit of the net pay will be made until the verification process has been completed.
Forward the Direct Deposit Authorization Form to the Controller’s Office, 202 Markel Hall. Incorrect forms
will be returned and the information must be resubmitted on a new form.
DO NOT CLOSE ANY ACCOUNT BEFORE CONTACTING THE CONTROLLER’S OFFICE FIRST.
